You can find that answer from your state bar's web page. For instance just type www.(your state)statebar.org in your city and you can get the information from there. But as far as I know, and each state has its own rules and regulations, the answer would closely fit 33 1/3 % of the settlement.

No, at most there is a prescribed fee for notarial fees and court awarded attorney's fees. Otherwise, an attorney can charge his clients as much as he believes that he's worth, i.e. the better the reputation of the lawyer - the more expensive he is.
Not usually, no. There's no laws against anyone in any profession making as much money as people are willing to pay for their services.
